Each of these is a document the company holds and can produce. They are listed because a diagnostic centre without them is not one you should give a blood sample to.
Radiation equipment is regulated separately from the laboratory. This is the permission to switch the machine on.
Needles, tubes and samples leave the building under a regulated disposal chain, not in a bin.

The lab is run by a private limited company, not a sole trader — the entity on your invoice is the entity that is accountable.
