A chest X-ray is one of the most frequently performed diagnostic imaging examinations in modern medicine. Whether requested for an unresolved cough, pre-employment fitness, or pre-surgery evaluation, it offers a rapid, non-invasive look at the heart, lungs, and thoracic cage.
When doctors order a chest X-ray
A physician typically advises a digital chest radiograph when evaluating:
- Persistent cough lasting more than 2–3 weeks, or cough with fever and sputum
- Breathlessness or wheezing that comes on suddenly or worsens on exertion
- Suspected pneumonia, bronchitis, or tuberculosis (TB) screening
- Chest pain or trauma to exclude rib fractures, pneumothorax, or pleural effusion
- Pre-operative anaesthesia clearance before surgical procedures
- Pre-employment and visa health clearances
Why "PA View" is the gold standard
Most chest X-ray prescriptions specify PA View (Posteroanterior). During a PA exposure:
- You stand facing the digital detector with your chest pressed against the plate.
- The X-ray beam enters through your back (posterior) and exits through the front (anterior).
- Placing the heart directly against the detector minimizes optical magnification, providing an accurate, undistorted measurement of cardiac size and cardiothoracic ratio.
An AP View (Anteroposterior) is reserved for bedridden or incapacitated patients where standing is impossible.
What about Lateral View?
A PA and Lateral (2-view) study is often requested when the radiologist or pulmonologist needs three-dimensional localization of a shadow, nodule, or retrocardiac fluid that sits hidden behind the heart silhouette on a flat PA view.
Radiation safety: Digital Radiography (DR) vs conventional film
Modern diagnostic centres utilize high-frequency Digital Radiography (DR) systems.
- Radiation exposure from a single digital chest PA radiograph is exceptionally low — roughly 0.02 to 0.1 mSv, which is comparable to about 10 days of normal natural background radiation.
- DR detectors require significantly lower radiation doses than old analogue chemical-film X-rays while producing ultra-crisp digital images that can be reviewed instantly on high-resolution PACS monitors.
Preparation and what happens during the test
- Fasting: No fasting is required. You can eat and drink normally.
- Clothing: You will be asked to remove upper-body garments with metallic buttons, chains, necklaces, or underwire bras, and wear a clean cotton gown.
- Breathing instruction: The radiographer will ask you to take a deep breath in and hold it for 2–3 seconds while the exposure is captured. Full lung expansion provides clear visualization of lung parenchyma and costophrenic angles.
- Pregnancy precaution: Always inform the radiographer if you are or might be pregnant. Lead shielding is used where necessary, or non-radiation alternatives (like ultrasound or MRI) are considered.
Reports and digital images are reviewed by our radiologist and delivered within 2 to 4 hours.
